Your First Visit
Please try and arrive at least 15 minutes prior to your appointment. Your first visit will be approximately 45 to 60 minutes. We encourage that you do bring your spouse or significant other on your first visit. Upon your visit you will have a brief patient overview. The doctor will go over your medical history and will do a complete exam. If you do need x-rays these are totally done at our clinic.
What To Bring To Your First Visit
To make your first visit as proficient and thorough as we can, kindly bring with you the following information with you on your first visit:
Driver license
Health Insurance card
Completed New Patient Form (see new patients page)
Diagnostic imaging such as X-rays or MRIs, if available
Medical history
Patient Forms
Please complete the new patient form and bring it with you to your first appointment.
